At this clinic, the first of the 2 part Pfizer vaccine will be administered by the Sullivan County Public Health Dept.
The clinic is for anyone 12 years of age or older. A person less that 18 years of age must have a parent/ guardian present at time of vaccination. Parents will be asked to sign permission forms for minor to receive vaccine.
It is requested that students/ parents bring some form identification for the student.
Please arrive no earlier than 15 minutes before your scheduled appointment time.
Return date for 2nd dose is Wednesday, June 30th. You will be automatically registered for your second dose and will receive a confirmation e-mail.
