At this clinic, the second of the 2 part Pfizer vaccine will be administered by the Sullivan County Public Health Dept.
The clinic is for anyone 12 years of age or older. A person less that 18 years of age must have a parent/ guardian present at time of vaccination. Parents will be asked to sign permission forms for minor to receive vaccine.
It is requested that students/ parents bring some form identification for the student.
Please arrive no earlier than 15 minutes before your scheduled appointment time.
